Through this memorandum, we are raising our twin demands to initiate disciplinary proceedings and simultaneously get a FIR filed under appropriate section of IPC against all the culprit public servants who along-with IPS No. 19921024,a 1992 batch UP cadre IPS Amitabh Thakur, presently posted as Joint Director – Civil Defense willfully & dishonestly cheated state government by act of forgery with intent to defraud public coffers by making False entries in official Vehicle Log book of said IPS. This refers to my RTI plea and subsequent reply by Surendra Singh Negi,PIO of Directorate of Civil Defense of Uttar Pradesh, sent to me vide letter no. 158/anu-1/121/2014 dated 30-01-2015. Vide this RTI reply, Negi has furnished me a leave application of Amitabh Thakur dated 10-04-14 which states that on 15-04-2014,16-04-2014 & 17-04-14, Amitabh Thakur was on leave and was off to New Delhi to file a Special Leave Petition & for other personal reasons. Vide this same RTI reply, Negi has furnished me a certified copy of Amitabh Thakur’s official Vehicle’s Log book, duly verified by IPS Amitabh Thakur which states that on 15-04-2014,16-04-2014 & 17-04-14, Amitabh Thakur boarded this official vehicle from his home for official purpose and on these dates the said vehicle ran 76 Kms.,56 Kms. & 61 Kms. respectively which is impossible as said public servant was on leave on these days , sitting in New Delhi some 500 odd Kms. away from Lucknow. This makes it clear that Amitabh has dishonestly cheated State Government by making false document ( Log book entries ) to support his claim for fuel of vehicle from state exchequer. This way Amitabh Thakur has willfully, and with intent to defraud committed this fraud , forgery and falsification of accounts by making false entries for 15-04-2014,16-04-2014 & 17-04-14 in his official vehicle Log book. Some other public servants may also be involved in this crime. So, we are raising our twin demands to initiate disciplinary proceedings and simultaneously get a FIR registered under appropriate section of IPC against all the culprit public servants who along-with IPS Amitabh Thakur willfully & dishonestly cheated state government by act of forgery with intent to defraud public coffers by making False entries in official Vehicle Log book of said IPS. Attachments : 1- Photocopy of RTI reply letter no. 158/anu-1/121/2014 dated 30-01-2015. 2- Photocopy of leave application of Amitabh Thakur dated 10-04-14. 3- Photocopy of Vehicle Log-book duly certified by Amitabh Thakur.
